10/30/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Ms. Broadnax upon arrival to the home setting. Upon arrival the home, Ms. Broadnax appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her afternoon. Ms. Broadnax discussed with QMHPA her afternoon was going okay and she was glad to have had the little break she did because it allowed her to search for things independently as QMHPA has been teaching her to do (i.e. housing). Intervention: QMHPA began the session by inquiring about Ms. Broadnaxâs current thoughts, feelings, and emotions. QMHPA reviewed with Ms. Broadnax the importance of being able to identify and utilize community resources. QMHPA prompted client to obtain her Resource Management Tool. QMHPA reviewed the Resource Management Tool with Ms. Broadnax. QMHPA engaged Ms. Broadnax in discussion surrounding a community integration activity. QMHPA assisted Ms. Broadnax on a community integration activity. The purpose of this activity was for Ms. Broadnax to identify resources which may be beneficial in the future. QMHPA reviewed the activity with Ms. Broadnax. QMHPA engaged Ms. Broadnax through active listening and asking open-ended questions. QMHPA praised Ms. Broadnax for a job well done. QMHPA informed Ms. Broadnax of the goal and objective for the following session. Response: Ms. Broadnax appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session. Ms. Broadnax identified her mental health as âokayâ. Ms. Broadnax stated that she had taken her medication. Ms. Broadnax was attentive and engaged during review of the importance of being able to identify and utilize community resources. Ms. Broadnax demonstrated the ability to obtain Resource Management Tool. Ms. Broadnax agreed to engage in a community integration activity. During the activity Ms. Broadnax was able to utilize community resources in an effort to identify other resources which may be beneficial in the future. Plan: QMHPA will continue to teach Ms. Broadnax to distinguish between wants versus needs and educate her regarding the impact of prioritizing through various skill building activities 1-2 times (Weekly/Monthly).
